PENANGANAN TERBAIK KESULITAN MAKAN BUKAN DENGAN PEMAKSAAN, MEMARAHI, MENGANCAM ATAU PEMBERIAN VITAMIN JANGKA PANJANG,
TETAPI KENALI DAN ATASI PENYEBABNYA (PALING SERING GANGGUAN SALURAN CERNA)
GEJALA SULIT MAKAN
Memuntahkan atau menyembur-nyemburkan makanan yang sudah masuk di mulut anak.
Makan berlama-lama (“dikemot”) dan memainkan makanan.
Sama sekali tidak mau memasukkan makanan ke dalam mulut.
Menumpahkan makanan, dan menepis suapan dari orangtua
Hanya mau makan makanan cair atau lumat
Kesulitan menghisap, mengunyah,menelan (Tidak mengunyah, langsung menelan makanan)
FAKTOR YANG MEMPENGARUHI KESULITAN MAKAN PADA ANAK
· GANGGUAN NAFSU MAKAN
Tampilan gangguan nafsu makan yang ringan berupa minum susu botol sering sisa, waktu minum ASI berkurang (sebelumnya 20 menit menjadi 10 menit), makan hanya sedikit atau mengeluarkan, menyembur-nyemburkan makanan atau menahan makanan di mulut terlalu lama (”dikemut”) Sedangkan gangguan yang lebih berat tampak anak menutup rapat mulutnya, menepis suapan orang tua atau tidak mau makan dan minum sama sekali.
· GANGGUAN PROSES MENELAN DAN MENGUNYAH
Keterlambatan makanan kasar dan berserat. Tidak bisa makan nasi tim saat usia 9 bulan, belum bisa makan nasi saat usia 1,5 tahun, tidak bisa makan sayur, daging sapi (empal) atau sayur berserat seperti kangkung. Proses makan terjadi mulai dari memasukkan makan dimulut, mengunyah dan menelan.
PENYEBAB KESULITAN MAKAN
· PALING SERING : GANGGUAN PENCERNAAN (karena ALERGI MAKANAN, INTOLERANSI MAKAN, COELIAC dll)
· Infeksi akut (Flu, batuk, pilek, panas, diare)
· Kelainan kronis SANGAT JARANG (TBC, Infeksi saluran kencing, keganasan, kelainan jantung bawaan, kelainan endokrin dan metabolik, dan gangguan neurologis (persarafan), Gangguan Psikologis
GANGGUAN SALURAN CERNA BERESIKO MENGAKIBATKAN KESULITAN MAKAN PADA ANAK
WASPADAI GANGGUAN PENCERNAAN
o Pada usia bayi : Sering muntah/kembung, sering “cegukan”, sering buang angin, sering “ngeden /mulet”, sering REWEL / GELISAH/COLIK terutama malam hari),
o Sering buang air besar (> 3 kali perhari) atau susah buang air besar ( ngeden, tidak BAB setiap hari, feses keras hitam atau hijau tua,kecil hitam spt ”tahi” kambing. Buang Air Besar bau menyengat.
o Lidah sering kotor (berpulau-pulau), timbul putih, sariawan, bibir kering, air liur berlebihan atau mulut berbau.
o Sering muntah, sering nyeri perut ringan dan hilang timbul, tidur malam sering ”nungging”
o Sejak lahir berat badan tidak pernah optimal atau berat badan kurang setelah umur 4-6 bulan.
GEJALA LAIN YANG MENYERTAI :
o Kulit sensitif, pada bayi sering timbul bintik atau bisul kemerahan terutama di pipi, telinga dan daerah yang tertutup popok. Kerak di daerah rambut. Kulit kering.
o Timbul bekas hitam seperti tergigit nyamuk. Sering menggosok mata dan telinga.
o Kepala,telapak kaki/tangan sering teraba hangat.Sering keringat berlebihan.
o Gigi mudah rusak atau berwarna kecoklatan dan keterlambatan pertumbuhan gigi
o Gangguan proses menelan atau mengunyah biasanya disertai oleh gangguan motorik kasar lainnya seperti terlambat bolak-balik (normal usia 4 bulan), terlambat duduk merangkak (normal 6-8 bulan), jalan jinjit, duduk bersimpuh leter “W”, terlambat berjalan, sering terburu-buru atau berlari, sering jatuh atau menabrak. Ciri lainnya biasanya disertai gejala anak tidak bisa diam, mulai dari overaktif hingga hiperaktif.

· GANGGUAN PERTUMBUHAN (BERAT BADAN DAN TINGGI BADAN KURANG & SULIT NAIK)
KESULITAN MAKAN PADA ANAK SERING BERKAITAN DENGAN GANGGUAN PERILAKU PADA ANAK
· GERAKAN MOTORIK BERLEBIHAN
usia <> 6 bulan bila digendong sering minta turun atau sering bergerak/sering menggerakkan kepala ke belakang-membentur benturkan kepala. Sering bergulung-gulung di kasur, menjatuhkan badan di kasur (“smackdown”}, sering memanjat. Perilaku “TOMBOY” pada anak perempuan
· GANGGUAN TIDUR (biasanya MALAM-PAGI) gelisah/bolak-balik ujung ke ujung, bila tidur posisi “nungging”, berbicara/tertawa/berteriak dlm tidur, sulit tidur, malam sering terbangun/duduk,mimpi buruk, “beradu gigi” atau gigi gemeretak (brushing)
· AGRESIF sering memukul kepala sendiri,orang atau benda di sekitarnya. Sering menggigit, menjilat, mencubit, menjambak (spt “gemes”)
· GANGGUAN KONSENTRASI : GANGGUAN KONSENTRASI DAN GANGGUAN BELAJAR : CEPAT BOSAN terhadap sesuatu aktifitas (kecuali menonton televisi, baca komik atau main game), TIDAK BISA BELAJAR LAMA, terburu-buru, tidak mau antri, TIDAK TELITI, sering kehilangan barang atau sering lupa, nilai pelajaran naik turun drastis. Nilai pelajaran tertentu baik, tapi pelajaran lain buruk. Sulit menyelesaikan pelajaran sekolah dengan baik.Sering mengobrol dan mengganggu teman saat pelajaran. ANAK TAMPAK CERDAS DAN PINTAR.
· GANGGUAN KOORDINASI DAN KESEIMBANGAN :
BOLAK-BALIK, DUDUK, MERANGKAK tidak sesuai usia. Berjalan sering terjatuh dan terburu-buru, sering menabrak, jalan jinjit, duduk leter W/kaki ke belakang. Jalan Pincang.
· KETERLAMBATAN BICARA : Tidak mengeluarkan kata umur <> 2 tahun membaik.
· IMPULSIF : banyak bicara/tertawa berlebihan, sering memotong pembicaraan orang lain)

CURICULUM VITAE :
Dr Widodo Judarwanto SpA, lahir di Surabaya. Sarjana kedokteran diperoleh dari Universitas Airlangga lulus tahun 1989, sedangkan Program Pendidikan Dokter Spesialis Anak diselesaikan tahun 1999 di Fakultas kedokteran Universitas Airlangga Rumah Sakit Dr Sutomo Surabaya. Pendidikan lanjutan dan berbagai kursus dalam dan luar negeri dalam bidang kesehatan anak terus dilakukan. Saat ini subspesialis kesehatan anak yang lebih diminati adalah “Allergy-Immunology” dan “Neurodevelopment Behaviour”.
Riwayat pekerjaan sebagai dokter umum secara formal bekerja di Puskesmas di propinsi Kalimantan Selatan selama 4 tahun. Setelah menyelesaikan pendidikan Dokter Spesialis Anak sempat sebentar mengabdikan diri di Rumah Sakit Dili Timor Timur, Saat ini bekerja di Neonatology Intensive Care Unit (NICU) dan ”Children Allergy Center” Rumah Sakit Bunda Jakarta, Jl Teuku cikditiro 28 Jakarta Pusat, serta di ”Allergy Asthma Therapy” dan ”Picky Eater Clinic” (Klinik Kesulitan makan Anak” di Children Family Clinic, Jl Rawasari Selatan 50 Cempaka Putih Jakarta Pusat.

Beberapa kegiatan ilmiah yang telah dilakukan telah menghasilkan beberapa karya ilmiah, termasuk karya ilmiah non popular. Selain rajin menulis masalah kesehatan anak di surat kabar atau majalah, juga menjadi konsultan kesehatan anak dalam sebuah media cetak.
